The itch is maddening. It is a deep, crawling sensation that scratching does not relieve. Scratching only replaces the itch with an intense burning pain.
Skin lesions also exhibit something called the Koebner phenomenon. Oral lichen planus requires a different focus. Borax is famously associated with oral sensitivity. It is heavily indicated when the inside of the mouth feels hot and tender, with aphthous-like ulcers that bleed very easily when touched by a toothbrush or food.
Merc Sol is another heavily used medicine for oral symptoms. The symptom picture here includes excessive salivation, especially at night, heavily coated tongues, and a distinct metallic taste in the mouth. The breath is often offensive, and the gums may feel spongy and painful.
When a patient has a clear history of suppressed grief—perhaps they lost a loved one and never truly cried, holding all their emotions inside—Natrum Mur is often the constitutional medicine that helps the system reset. These patients often crave salty foods and find their skin symptoms worsen in the heat of the sun.
Realistic timelines
I have to be brutally honest with my patients about time. Lichen planus is incredibly stubborn. If you are looking for a lotion that will make the bumps disappear in ten days, homeopathy is not going to provide that.
A realistic timeline for constitutional homeopathic treatment is anywhere from six to eighteen months. Progress is rarely a straight line. You will have weeks where the itching settles down and the oral patches fade, followed by a sudden flare-up. These flare-ups almost always coincide with a bad week at work, a bout of poor sleep, or a viral fever that puts the immune system on high alert again.
You must also manage your diet carefully during this time. For oral lichen planus, you have to temporarily abandon spicy curries, raw onions, hard crispy snacks, and acidic fruits. Your mouth needs a completely bland environment to heal.
There are also times when conventional medicine is strictly necessary. If erosive oral lichen planus becomes so severe that a patient cannot swallow water, or if an oral ulcer refuses to heal for weeks, they must see a conventional specialist. Persistent, changing ulcers always require evaluation and sometimes a biopsy to rule out other, more dangerous pathologies. Homeopathy is a gentle, supportive system, but it does not replace urgent medical diagnostics.
